    • How frequent are the seizures

  • Pertinent physical exam findings
  • Recent pertinent hospitalizations, labs, radiology
  • Current medications managed
  • Frequency of visits/monitoring
  • Presence of intellectual disability, if applicable
History
  • Previous treatments, medication trials, diagnostic imaging results

  • Durable medical equipment for any equipment or devices
