T&A Triage with or without Preoperative Polysomnography Clinical Pathway – Inpatient and Outpatient Specialty Care

ASC and CHOP Day Surgery Discharge Criteria

ASC and CHOP Day Surgery patients are ambulatory patients who may be discharged home according to home readiness criteria approved by the medical staff/responsible provider and with a Recovery Score of 6 (or at baseline status).

Vital Signs	Temperature, HR, BP Stable/within normal limits or at baseline
Consciousness	Awake or back to baseline level of consciousness
Airway/Respiration	Patent airway with protective reflexes Adequate ventilation, normal respiratory rate No evidence of respiratory distress, baseline respiratory status
Bleeding	No excessive surgical drainage or bleeding
Movement	Moving all extremities, back to baseline level of activity
PO	Adequately hydrated Emesis No more than 2 emetic episodes and No emesis within 30 minutes of discharge
Pain	Pain scales Faces (> 3 y/o, < 8 y/o) Numeric 0-10 (> 8 y/o)
Discharge	Parent/legal guardian/responsible adult is present to escort the patient home, include: Appropriate prescriptions Discharge instructions Hospital and personnel phone numbers
