Weaning Recommendations
- Evaluate ability to wean/discontinue if underlying illness has resolved, and no later than 5-7 days after initiation of therapy to determine if:
- Underlying disease process is improving
AND - Behaviors are well controlled
- Underlying disease process is improving
- Discontinue medication as soon as possible if adverse drug event is noted
- There are no specific guidelines for weaning these medications. Discuss a plan for weaning with PICU provider, clinical pharmacy and/or Psychiatry.
- Begin weaning as soon as possible with PRNs available while noting clinical status and pharmacological properties of the specific medication being weaned
- Generally, duration of weaning should not exceed duration of antipsychotic therapy
