Patient/Family Meeting
As part of our Promise of Partnership©, one of CHOP’s core values of family-centered care, we constantly seek to improve how we communicate and interact with patients, families and one another. Family meetings are one key way we communicate with families and colleagues. We have developed the following tools to help clinicians and family members better prepare for family meetings and facilitate communication during these meetings.
Tips for Patient/Family Meeting
- Determine if family meeting is needed
- Team members include representative from medical team, social work and BHIP
- Social Work will coordinate and lead the family meeting
- Goals of the meeting:
- Discuss medical and BH updates and recommendations
- Mutual agreement between providers and patient/family
- Timeline established for discharge
