Behavioral Concerns, Child Undergoing EEG Monitoring Clinical Pathway — Inpatient and Outpatient Specialty Care

9S-NSU-EMU Plan of Care Communication

Admission to 9 South Neuroscience Unit/EMU is the preferred location for children with behavioral health concerns given presence of specialized nursing, child life specialists (CLS), ceiling cameras, and EEG technologists. Review with nursing leadership and physician/nursing flow leaders as indicated.

Communication Between RN, EEG Technologist, and CLS
  • EEG technologist contacts RN before seeing a child with an identified behavioral concern.
  • RN informs EEG technologist of plan, including CLS plan of care and then works with EEG technologist to perform the procedure.
  • Box in EMU treatment room contains:
    • Chewy tubes, spiky balls, stress balls, fidget toys, I Spy books, stickers, noise-canceling headphones
  • CLS has access to many developmentally appropriate play and distraction items.
EMU Interdisciplinary Team Communication
  • Team Rounds/Nursing Huddles
    • Review/discuss CLS plan of care at nursing team huddles and team rounds to ensure consistent communication among team members, family, and child.
  • Consultations
    • Behavioral Health Integrated Program (BHIP) as indicated.
  • Agitation Escalation Plan
    1. Team huddle regarding plan.
    2. Consider whether safety observation is needed.
    3. Review Medication Options.
    4. Review restraint indications:
    5. Discontinue the EEG: In rare cases, the EEG may need to be discontinued to de-escalate the child and keep everyone safe.
Discharge Documentation
  • Inform family that the CLS plan of care is accessible in MyCHOP to utilize in the future if needed.
  • If pre-medications were used, then include notes regarding effectiveness in the discharge summary and care coordination encounter to outpatient clinician to aid future care.
